4 Units of Katana

Sometimes patients come to us needing extensive dental work, and with a history of extensive dental work. This fine gentleman came to us with a fractured tooth on #3. After having restored it with an implant (it was not restorable, sadly) he asked what could be done about his broken and breaking teeth in the front.

We discussed the possibility of a full mouth rehab, but he insisted at his age that was too much to go through. We came up with a treatment plan for him that balanced his needs and his function, giving him the ability to confidently smile again.

Before – Smile

We opted to restore the 4 upper incisors, after radiographic review and caries removal. We took photos, sent off for a digital waxup, and were ready to get started!

This case was done entirely in-house using CEREC. Due to how much his bite is worn down, and how much his guidance was on those anterior teeth, we opted to use Katana ONE by Kuraray Noritake for its kindness to opposing dentition and strength.

The teeth were prepped through a prep guide, and the preps were scanned into CEREC (PrimeScan). The diagnostic waxup was scanned as a bio-copy. Due to the limitations of only restoring 4 teeth, shade selection was difficult! We opted to use shade D3 in hopes it would blend with his natural dentition.

The restorations were designed, milled, and characterized in the green state prior to sintering. They were sintered, all together, in the SpeedFire which thankfully only takes 18 minuets! After sintering, they were additionally characterized with diamond burs (CEREC CAD-CAM finishing kit) and glazed using Kuraray’s CERABIEN™ ZR paste glaze system.

Sadly, I didn’t do a perfect job the first time painting the glaze, and had to glaze them again to get an even coat. After a try-in we still didn’t love the shade, so I used the A+ stain and glazed them one more time, before finally seating them using RelyX RMGI Luting Cement. Huge results for a ‘one day’ smile makeover!

After – 2 Weeks Later

We brought him back in a week to adjust his bite, and he told us how much he loved his teeth. He said he hasn’t smiled in decades, and was just getting used to it again! Thankfully, he smiled for the after photos for us!
