“Please let me save those teeth!”

We get every type of consult under the sun at ADIC. Most of the time, I am the one telling patients we need to part ways with a few remaining teeth to do implants, which is always a tough conversation. That is how we do ‘All-On-4’ procedures – we use the good bone that the remaining teeth preserved to hide the implants and to support the prosthetics.

But this patient came to us because she wanted implants! She hated the dentist and wanted to get it all done at once. She wanted to wake up with a new smile. At first glance of her x-rays, hybrids were an option. As we kept thinking about it, 4-10 were all in good shape. That’s a lot of teeth to take out just to work on the other side. She also had enough bone for implants elsewhere… so we came up with a radical treatment plan. We were going to make a temporary bridge in Zirconia from 12-15 to cover up implants placed at 12 and 14, do a RCT/Post/Crown on #11, and do an immediate implant and sinus lift at #3. She could wear the temporary, but strong, bridge while the implants heal, then we can restore the implants. We could do all of this for her while she slept, so she could wake up with a new smile!

We offered her this option, but realized that she still wanted to improve her smile more than just replacing the missing teeth. No problem, we decided to include veneers from 6-10 to improve her smile! Sadly, #11 was ‘locked in’ with her lower canine, so I was going to be forced to BioCopy that tooth to preserve the occlusion. So while she slept, we prepped for the veneers too. We couldn’t do the same day permanent veneers since our CEREC was busy with her temporary Katana bridge, but we made her locked on temps in Luxatmp, milled and glazed the veneers over the weekend, and brought her back in a week to seat them!

She slept like a baby, and said this was without a doubt, the best dental experience she has ever had! Sedation is such a powerful tool that enables us to do more dental work faster, while making patients more comfortable. Sedation isn’t just for hybrids or surgery. We routinely do sedation for root canals and crowns, too. For this patient, it took us from 8am until 1pm to get all of her work done, which was: 2 extractions, 3 implant placements, 2 sinus lifts, one root canal, two posts, a pulpotomy, two core buildups, 5 veneer preps, one crown prep, and a temporary bridge. Phew, what a morning!
