Whole Mouth Rehab — TMJ

This awesome patient came to us with an odd complaint. After having had some dental work done in India, he was having TMJ pain and couldn’t eat!

We looked at his teeth and they were worn flat! He also had a significant amount of wear and recession on the outside surfaces of the teeth. His PAN showed that he did have noticeable degradation of the joint, and his CR and MI positions were off – or his bite wasn’t good for his joints.

Before and After Bite

We did splint therapy to relax the muscles, botox to reduce the pain, and took some scans in his proper CR position. Due to the failing bridge on the top, we opted to do an upper FP1 hybrid and the lower arch would be remade with crowns to level the bite and put back some cusps for chewing capacity.

He came in and was put to sleep. We did the entire lower arch crowns and placed his two implants on the bottom. A week later, we did the same on the top, placing his implants and 3D printing him a temporary. Due to how thin the prosthetic was, we were limited on options while he healed – so he just had to eat almost a liquid diet!

After 3 months, we made his final upper prosthetic. We had a bit more gum giveback than we anticipated, so we made the prosthetic have a tiny bit of pink gums to make it look better.

Now he can chew again! All of his teeth are in proper lateral group function, anterior guidance, and solid MIP which makes his TMJ happy. Pain free since we did his upper temp!

Final Ceramics

He loves the smile, but mostly is thankful to chew again! We love the ceramics on the upper – this is an FP1/FP2 Zirconia hybrid made by @SynrgLab and we could not be happier with the outcome!
